CMS is deploying $ 10 Billion per year for five (5) years to states with strong rural health transformation plans aimed at improving access, quality, patient engagement, and long-term provider sustainability.  Funding supports rural collaboratives and modern technology adoption, including interoperability, analytics, telehealth, AI, and virtual care.

New York received $212.1M in Year 1 alone, launching a statewide Rural Health Transformation Program (RHTP) built around four initiative tracks:

  1. Rural Community Health Integration — plans for collaboratives and specific initiatives
  2. for Technology‑enabled primary care — expanding digital tools, AI, and advanced care models
  3. Rural workforce development — pipelines to recruit, train, and retain rural clinicians
  4. Technology innovation & cybersecurity — strengthening telehealth, data exchange, analytics, and security

Why it matters:

  • $76M will be distributed for Track 1; applications have been submitted
  • Calls for applications for $136 Million more for Tracks 2, 3 and 4 will be released any day
  • All year one funds to be allocated by September 30

 New York’s RHTP aims to close rural health gaps by improving coordinated care access, modernizing primary care, stabilizing the workforce, and upgrading digital infrastructure statewide.
